SORE THROAT  
DECREASED APPETITE 
BREATHELESSNESS SINCE 4 DAYS

HISTORY OF PRESENT ILLNESS: 
PATIENT WAS APPARENTLY ASYMPTOMATIC 4 DAYS BACK HE DEVELOPED FEVER WHICH WAS INSIDIOUS IN ONSET INTERMITTENT TYPE WITH CHILLS


HISTORY OF PAST ILLNESS: 

PATIENT HAD A LIVER RUPTURE 5 YRS BACK AND 
PATIENT UNDERWENT APPENDICECTOMY

PATIENT HAS DIABETES MELLITUS 
HYPERTENSION

NO PALLOR ICTERUS CYANOSIS LYMPHADENOPATHY CLUBBING OEDEMA 
MILD DEHYDRATION

RESPIRATORY RATE: 16 /MIN

CVS 
NO THRILLS, MURMURS 

RESPIRATORY SYSTEM 
NO DYSPNOEA WHEEZE  
POSITION OF TRACHEA : CENTRAL 
BREATH SOUNDS : VESICULAR
